The information we collect
When you contact us
If you use the enquiry form or email us, we collect your name, telephone number, email address and anything you choose to tell us about your symptoms or reason for contacting us.
When you are a patient
To provide care we hold clinical records — your health history, examination and assessment findings, any x-rays taken where medically required, and the treatment provided. This is information about your health, which is a special category of personal data.
Why we use it, and our lawful basis
- To respond to your enquiry and arrange appointments — on the basis of your consent and our legitimate interest in answering you.
- To provide chiropractic care and keep proper clinical records — on the basis of the provision of health care (UK GDPR Article 9(2)(h)) and our legal and professional obligations as a General Chiropractic Council registered practice.
- To meet legal, insurance and regulatory requirements.
Who we share it with
We do not sell your information. We may share it, only where appropriate:
- with software providers that operate our booking, records or email systems on our behalf (for example our practice-management and online-booking provider), who act as our processors under contract;
- with your GP, an insurer or another healthcare professional, where you ask us to or where it is necessary for your care;
- where we are required to by law or by our regulator.
How long we keep it
Enquiries that do not lead to care are kept for [period to confirm] and then deleted. Clinical records are kept for the period required by professional and legal guidance [confirm retention period — for example eight years for adults, and longer for children].
Your rights
You have the right to ask for a copy of your information, to have it corrected, to ask us to delete it, and to object to or restrict how we use it, subject to our legal and professional obligations. To exercise any of these, email us at Hertford@healthchiropractic.com.
